IUD/Intra Uterine Devices sumber theasianparent Para ibu yang kerap merasa repot saat harus meminum pil KB secara teratur biasanya lebih memilih alat kontrasepsi berupa KB spiral atau IUD. Secara umum, KB spiral terbagi menjadi dua jenis, yakni IUD tembaga yang terdiri dari berbagai merek, seperti Nova T dan Andalan, serta IUD hormonal seperti merk Mirena dan lain-lain. Lantas, apa perbedaan IUD Nova T dan Andalan? IUD Intra Uterine Devices merupakan kontrasepsi yang dimasukkan ke dalam rahim yang terbuat dari bahan polietilen dengan atau tanpa metal untuk steroid. IUD sangat efektif untuk menjarangkan kehamilan dibandingkan dengan metode kontrasepsi jangka panjang lainnya seperti implan, tubektomi, dan vasektomi. IUD merupakan metode kontrasepsi jangka panjang yang paling banyak digunakan dalam program KB di Indonesia. Pengguna IUD di Indonesia mencapai 22,6% dari semua pengguna metode kontrasepsi.[1] Tentang IUD Andalan IUD Andalan adalah kontrasepsi berbentuk T yang diletakkan di dalam rahim untuk mengacaukan cara kerja sperma sehingga dapat mencegah pembuahan sel telur. IUD atau yang sering disebut spiral, dapat memberikan perlindungan dari kehamilan selama bertahun-tahun, dengan pilihan jangka waktu antara 4, 5, hingga 10 tahun tergantung dari tipe IUD yang digunakan. Terdapat beberapa jenis IUD Andalan, yakni Tcu 380 A Copper T, Silverline Cu 380 Ag, TCu 380 A Postpartum, Sleek Cu 375, Tcu 380 A Safe Load, dan Silverline Cu 200 Ag. Tentang IUD Nova T IUD Nova T merupakan produk KB spiral buatan pabrikan farmasi asal Jerman, Bayer. NOVA T 380 adalah alat kontrasepsi dalam rahim yang terbuat dari polietilen dan dililit dengan kawat tembaga dengan inti perak. Luas permukaan tembaga adalah 380 mm2. Badan polietilen, dibentuk sebagai T yang dimodifikasi dengan kandungan barium sulfat. Benang pelepasan, berpigmen dengan oksida besi melekat pada alas dari lengan vertikal T. IUD Nova T & IUD Andalan Nah, lantas apa perbedaan dari IUD Nova T dan Andalan? Biasanya jenis IUD yang kerap dibandingkan dengan Nova T adalah jenis IUD Andalan Tcu 380 A alias Copper T. Copper-T terbuat dari bahan polietilen berbentuk huruf T dengan tambahan bahan Barium Sulfat dibalut tembaga sebanyak 176 mg tembaga luas permukaan 380 ± 23m2. IUD Nova-T mempunyai 200 mm2 kawat halus tembaga dengan bagian lengan fleksibel dan ujung tumpul sehingga tidak menimbulkan luka pada jaringan setempat pada saat dipasang. Berdasarkan teori pada William Obstetri, dikatakan bahwa penggunaan IUD jenis copper-T ini menyebabkan banyak perdarahan. Hal ini kemungkinan dikarenakan karena bentuk dan ukuran dari Copper-T yang lebih kaku dan tidak fleksibel seperti halnya Nova-T. Pada kenyataannya bentuk dan ukuran antara Copper-T dan Nova-T memang terdapat perbedaan. Copper-T memiliki bentuk yang lebih kaku dan ukuran yang lebih besar dibandingkan dengan Nova-T. Nova-T memiliki bentuk ujung yang lebih tumpul dibandingkan dengan Copper-T sehingga perlukaan yang ditimbulkan Nova-T tidak sedalam Copper-T. Selain itu ukuran Nova-T juga lebih kecil dibandingkan Copper-T. Sehingga luas perlukaan yang dihasilkan oleh Nova-T lebih kecil dan tidak menimbulkan banyak perdarahan. Sebaliknya, pada Copper-T perdarahan yang terjadi saat haid volumenya mencapai 60-80 ml per daur. Nova-T terbuat dari bahan polietilen yang lebih elastis dibandingkan Copper-T, serta lilitan tembaga yang terdapat pada Nova-T lebih sedikit yaitu 200 mm2 sedangkan pada Copper-T lebih banyak mengandung lilitan tembaga yaitu 380 mm2. Elastisitas tersebut sangat berpengaruh terhadap perlukaan yang dihasilkan. Pada Nova-T yang memiliki elastisitas lebih hanya menyebabkan sedikit perlukaan sehingga tidak banyak menimbulkan perdarahan.[2] Meski ada perbedaan mendasar antara kedua merek KB spiral non-hormonal ini, secara umum fungsi keduanya sebenarnya sama saja, yaitu untuk mencegah terjadinya kehamilan. Secara cara kerja pun sama, yakni menghambat gerakan sperma untuk masuk ke rahim, mengganggu proses pembuahan, dan mencegah penempelan telur di rahim. Di samping itu, dari segi harga juga IUD Nova T biasanya terpaut lebih mahal dibandingkan IUD Andalan. Sebagai referensi, harga IUD Nova T saat ini berkisar antara Rp335 ribu hingga Rp425 ribu. Abstract In a comparative use-effectiveness study, 855 Nova T and 883 TCu 200 Ag devices were inserted in a randomised sequence. By the end of the first year the gross rate of unplanned pregnancy was per 100 users of Nova T and per 100 users of TCu. After 2 years the pregnancy rates for Nova T and TCu were and per 100 users, respectively. Although there was no statistically significant difference between the rates for the two devices, they suggest a higher contraceptive efficacy for Nova T. PIP Between February 1982 and February 1983, 2 family planning clinics in Hungary enrolled 1738 subjects for a study comparing contraceptive efficacy and clinical performance of 2 types of IUD. In the comparative use-effectiveness study, 855 Nova T and 883 TCu 200 Ag devices were inserted in a randomized sequence. Subjects were advised to return to the clinic for a check-up at 1, 3, 6, 12, and 24 months after insertion. 40% of the women were IUD users before entering the study. In different age categories of Nova T users the pregnancy rate varied from among those under 25 to in those over 35. For Copper T users, these figures were under 25 and over 35. Abstract A randomized comparative trial on the clinical performance of two copper-releasing IUDs Nova-T and Copper-T-200 was performed simultaneously in Denmark, Finland and Sweden. After three years the cumulative pregnancy rate was for Nova-T and for Copper-T. This difference in efficacy was statistically significant p less than Copper-T had a lower expulsion rate p less than in the total series but not in postmenstrual insertions. Differences between the two devices in other termination rates were not statistically significant. Analysis according to age and parity demonstrated that the pregnancy rate of Nova-T was lower than that of Copper-T in every age and parity group. The pregnancy rate of Nova-T was not effected by age or parity whereas the pregnancy rate and the expulsion rate of Copper-T decreased with increasing age and parity. The removal rate because of infection decreased markedly after the first year of use for both devices. The cumulative rate of removals for infection during the three years of use was not significantly correlated to age and there was no correlation to parity. The continuation rates increased with age and parity. The continuation rates of nulli- and primiparous women were almost identical with both devices and lower than continuation rates of women with 2 or three or more children. Only 11% of the women were lost to follow-up during 36 months. Nova-T had superior effectiveness in preventing undesired pregnancies when compared with Copper-T. The performance of Nova-T is less affected by age and parity then the performance of Copper-T. The silver core in copper wire gives a prolongation of the life-span for Nova-T.
